2. Complete New Patient Intake
After booking your appointment, you’ll receive our New Patient Intake Form via email. Please complete it at least 48 hours before your scheduled visit.
3. Payment Made After Appointment
Please note that we’re an out-of-network clinic. We can provide a super bill after you pay for your visit.
4. Note Our 48 Hour Cancellation Policy
If you miss an appointment without providing 48 hours’ notice, we will ask that you either pay a cancellation fee of $50 immediately, or you may place credit card information on file in order to reserve an appointment in the future.
This credit card will be charged in the event of future cancellations (with notification to you, of course!)
